   COVID-19 Grants Certification
 

Background

Compensation is available to eligible K-12 classroom teachers and support staff employed by or
assigned to regular and continuous work under contract by this district if they meet eligibility
requirements. Requirements are that during the 2019-2020 school year they supported the
continued learning of students during the closure due to the Covid-19 pandemic. Pursuant to
Public Act 166 of 2020, staff members that meet the eligibility requirements listed on the
Michigan Department of Treasury website will receive this compensation directly from the state.

We applaud the efforts of staff and the district is pleased to assist in coordinating these benefits
for the efforts of our staff that went beyond due to needs of our students and community during
the pandemic. Thank you for all you do within the District.

District Responsibility

The district will collect Form 5734 titled Teacher and School Support Staff COVID-19 Grant
Certificate from each staff member deemed eligible. Under this Act, the District will report the
information from the forms to the Department of Treasury. The deadline for collection of the
form is no later than December 4, 2020.

Staff Responsibility

Teachers and support staff members are required to complete Form 5734 “School Support Staff
COVID-19 Grant Certificate”. The final date for submission by delivering the form to the
Administration Office is December 4, 2020. A teacher PIC number and the narrative for
supporting the claim is required on the form. The form emailed to each staff person is also
available in the business office. All eligible staff to receive the compensation must complete the
form.

Grant Payments

Compensation amounts vary based on the employee’s position and FTE during the 2019-2020
school year.


Teachers who work full time and are calculated as 1.0 FTE will receive up to $500 and those that
work less than 1.0 FTE shall receive the proration of the FTE applied amount.


Support staff who work full time and are calculated as 1.0 FTE will receive up to $250 and an
eligible support staff whose work is calculated as less than 1.0 FTE shall receive that proration of
the FTE applied amount.


Payments will be disbursed via check on or about February 25, 2021 to the address included on
your forms.
